DID YOU RECEIVE NOTICE OF A DATA BREACH FROM A2Z DIAGNOSTICS AND SUFFERED FRAUD AS A RESULT?
CaseyGerry is investigating a data security incident reported by A2Z Diagnostics (A2Z), a lab in New Jersey performing specialized diagnostic testing.
According to the notice of breach on the company website, A2Z recently reported a data security breach confirming that employee email accounts had been accessed by an unauthorized third-party between February 2, 2021 and April 2, 2021. The breach was discovered on June 28, 2021. The compromised data includes personal and protected health information including:
- Names
- Social Security number
- Date of birth
- Driver’s License or State Identification number
- Medical Diagnosis or Clinical Information
- Treatment Type or Location
- Doctor Name
- Health insurance information and/or
- Medical Procedure information.
