TriValley Primary Care, a Pennsylvania-based health care provider, has reported a data breach that compromised patients’ personal and health-related information.
According to the notice of breach filed, the company discovered the cyber security incident on October 11, 2021. Through their investigations, they learned that an unauthorized party gained access to their patient data. The report states that they are not aware of when their system was breached or how long the unauthorized party maintained access to their network.
The HIPPA Journal reported that at least 57,000 patients may have been affected by this breach.
Exposed information includes:
- Name
- Gender
- Mailing address and email address
- Date of birth
- Social Security Number
Health information such as:
- Medical history
- Service dates
- Lab information
- Diagnosis and treatment
- Prescription information
And financial information such as health insurance policy, claim information and more.
